Paperwork On your first visit we will ask you to fill out some brief paperwork concerning your medical history, which gives us important information about yourself and your past medical conditions. We will also verify insurance benefits under your health insurance plan, coverage under your employer'’s Workers’ Compensation carrier for work-related injuries, or your Personal Injury Protection (PIP) coverage for auto related injuries. Exam You will receive a complete examination, which will include all Orthopedic, Neurological, and Musculoskeletal components appropriate for the areas of complaint. A thorough examination will allow the doctor to assess whether there is any abnormal motion or position of the bones, abnormal nervous system function, abnormal muscle function, abnormal soft tissue function, or other abnormal function of the body. Your condition may require on site x-rays to further evaluate possible structural imbalance, which may contribute, to your condition. An x-ray serves as a "map" of your backbone allowing us to pinpoint diagnosis and treatments. Report of Findings (ROF) During your personal Report of Findings, Dr. Derieg will discuss with you his exam and x-ray findings.
